Position Summary:
We are seeking a dedicated and compassionate Family Physician to join our growing healthcare team. The ideal candidate will have a broad scope of clinical expertise to provide comprehensive, preventative, and acute care to patients of all ages. This role requires a commitment to quality patient care, strong communication skills, and the ability to work collaboratively within a multidisciplinary team-based practice model.
Duties & Responsibilities:
Provide Comprehensive Outpatient Care
- Evaluate, diagnose, and manage acute and chronic health conditions across all age groups.
- Deliver preventative care, including wellness exams, immunizations, cancer screenings, and lifestyle counseling.
- Perform minor office procedures as appropriate to the scope of practice (e.g., suturing, skin biopsies, joint injections, etc.).
Care Coordination & Chronic Disease Management
- Create individualized treatment plans for patients with chronic conditions such as diabetes, hypertension, COPD, and heart disease.
- Collaborate with specialists, care coordinators, and other healthcare professionals to manage referrals and ensure continuity of care.
- Follow up with patients to monitor progress and adherence to care plans.
Documentation and Compliance
- Accurately document all patient encounters and procedures in the clinics EMR system.
- Ensure timely submission of visit data and compliance with regulatory and organizational standards.
Professional Development and Leadership
- Stay current with developments in evidence-based family medicine and primary care practices.
- Participate in continuing education and professional development opportunities.
- Serve as a clinical resource and mentor to advanced practice providers (APPs) and other team members.

- Comfortably manage diverse patient populations in a fast-paced rural clinic environment.
Minimum Qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.
- Board-certified or board-eligible in Family Medicine.
- Active or eligible for medical licensure in states we serve.
Experience:
- Prior experience working in rural or underserved healthcare settings preferred.
- Demonstrated ability to manage a diverse patient population and collaborate across care teams.
- Proven ability to work in a productivity-based model where performance drives earning potential.
Education:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
